This festival has been declared to be of Tourist Interest by the Provincial Council of Málaga.

The earliest pilgrimages in honour of Our Lady of Cabeza (Virgen de la Cabeza) were performed for the foundation of the Cofradía de la Cabeza Brotherhood in the 16th century, as recorded in the Archives of the Real Santuario de la Virgen de la Cabeza de Andújar. These pilgrimages were later resumed in the 19th century.

The festivities now start on Friday night, but the the Romería (Pilgrimage), the most important event, takes place on Saturday. During this procession, Our Lady and San Isidro images are carried on adorned carts pulled by oxen all the way from Teba to the banks of the Guadalteba River. This is where campers and pilgrims will settle, next to kiosks and fair attractions, ready to enjoy concerts and other musical performances until dawn. The traditional Romera Mass is celebrated on Sunday, and it is customary to relish the landscape and delightful family meals.

Due to the increasing number of visitors, in 2018 the authorities approved and implemented some regulatory measures for environmental preservation by prioritizing the natural setting.
